Below you will find a form that your chapter can use to report on the success of your IU Cares Month project. Please complete and submit this form as soon as your project ends! The IU Cares Month Success form will also be available online at in the next few weeks.
Directions for Submitting Your Form
STEP 1: Complete all information requested on the form below.
- You are welcome to include attachments (optional). Please label all photographs with names and include the name of the photographer for publishing purposes.
- PLEASE...use a staple or paperclip to bind your entry and attachments if sending by mail.
